Boyd Longevity Care ("BLC") provides healthcare services through secure telehealth technology. By participating in telehealth services, patients acknowledge and agree to the following terms.
What is Telehealth?
Telehealth allows healthcare services to be delivered using electronic communications, including video conferencing, telephone communications, secure messaging, and patient portals.
Benefits of Telehealth
Telehealth may provide:
Improved access to care
Increased convenience
Reduced travel time and costs
Timely follow-up and monitoring
Access to healthcare services from home or another approved location
Potential Risks
Potential risks of telehealth include:
Technical failures or interruptions
Delays due to equipment or connectivity issues
Unauthorized access despite reasonable security measures
Limitations associated with the inability to perform a hands-on physical examination
Patient Responsibilities
Patients agree to:
Provide accurate health information
Verify their identity when requested
Participate from a private location whenever possible
Use secure internet connections and devices
Notify Boyd Longevity Care of any changes in contact information

Telehealth services are not appropriate for medical emergencies.
If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.
Consent
