MEMBERSHIP AGREEMENT

Effective Date: February 6, 2026

This Membership Agreement ("Agreement") outlines the terms and conditions of participation in membership programs offered by Boyd Longevity Care ("BLC," "we," "our," or "us").

By enrolling in a membership program, you agree to the terms described below.

Membership Services

Membership programs may include:

  • Initial and follow-up telehealth consultations

  • Ongoing provider support

  • Treatment monitoring

  • Prescription management when medically appropriate

  • Educational resources and wellness guidance

Specific membership benefits may vary by program.

Self-Pay Practice

Boyd Longevity Care is a self-pay practice.

Insurance is not accepted. Membership fees do not guarantee insurance reimbursement.

Laboratory testing, medications, pharmacy fees, and shipping costs may be billed separately unless otherwise stated.

Medical Eligibility

Enrollment in a membership does not guarantee approval for medications, therapies, or treatment plans.

All treatments are subject to clinical evaluation and medical appropriateness as determined by the provider.

Membership Fees

Membership fees are disclosed prior to enrollment.

Fees may be charged on a recurring basis according to the selected membership plan.

By enrolling, you authorize Boyd Longevity Care to process payments using the payment method provided.

Cancellations

Members may cancel their membership at any time by providing written notice to Boyd Longevity Care.

Cancellation requests must be submitted prior to the next billing cycle to avoid future charges.

Services already rendered and fees already charged are non-refundable except where required by law.

Refund Policy

Consultation fees, completed services, prescription processing fees, pharmacy charges, and shipped medications are generally non-refundable.

Refund requests will be reviewed on a case-by-case basis.

Medication Orders

Prescription medications are ordered only when medically appropriate.

Once a prescription has been submitted to a pharmacy, medication orders generally cannot be canceled or refunded.

Patients are responsible for reviewing medication instructions and reporting side effects promptly.

Patient Responsibilities

Members agree to:

  • Provide accurate health information

  • Attend scheduled appointments

  • Participate in recommended follow-up care

  • Report side effects or medical concerns promptly

  • Maintain updated contact information

No Guarantee of Results

Individual outcomes vary.

Boyd Longevity Care does not guarantee specific weight loss, health improvements, laboratory results, symptom improvement, or treatment outcomes.

Termination of Membership

Boyd Longevity Care reserves the right to suspend or terminate membership if:

  • False information is provided

  • Membership terms are violated

  • Treatment is no longer medically appropriate

  • Safety concerns arise

Changes to Membership Programs

Boyd Longevity Care may modify membership pricing, benefits, or program structure upon reasonable notice.
